Mobile Social Work Team goes to rural areas of Verkhneuslonsky municipal district
Mobile Social Work Team – an innovative social service technology that is implemented in work with senior citizens and people with disabilities at home by specialists of Home Care Department of the Complex Social Service Centre in municipal district of Verkhny Uslon.
The team can accept referrals to provide an assessment where a person’s disability or illness means that they need help to manage any aspect of day to day life. Social workers make assessments in co-operation with an individual and their careers along with other professionals and organizations who may be involved.
The team work with people living at home, in residential care or supported housing or who are about to be discharged from hospital. We offer our social work skills to help people and their careers make essential changes to their lifestyles, whatever the cause.
